Ketamine, an NMDA receptor antagonist, produces rapid antidepressant effects in subanesthetic doses administered intravenously or intranasally, with benefits lasting days to weeks. Treatment-resistant depression persists after at least two adequate antidepressant trials and is linked to functional impairment, prolonged emotional distress, and increased suicide risk. Early evidence suggests that ketamine-assisted psychotherapy may reduce relapses, improve functioning, and extend benefits, representing a paradigm shift by combining rapid symptom relief with positive impacts on quality of life and emotional resilience. This synergistic pharmacological and psychotherapeutic approach offers a promising strategy for treatment-resistant depression and may redefine its clinical management.
