A 37-year-old man with longstanding major depressive disorder, post-traumatic stress disorder, and social anxiety disorder who had not responded well to multiple medications experienced meaningful improvements in depression and anxiety after starting low-dose sublingual ketamine troches. He was also able to taper off and stop four psychotropic medications. No adverse effects occurred. This case suggests that sublingual ketamine may help reduce symptoms and simplify medication regimens in complex, treatment-resistant depression.
