Ketamine, known for over 60 years as a dissociative anesthetic, has a unique multimodal pharmacological profile beyond classic NMDA antagonism. This review traces its clinical evolution from initial use in anesthesiology and pain medicine to a rapid-acting antidepressant for treatment-resistant depression (TRD) and acute suicidality. Three key eras were identified: 1970–2000 as an anesthetic in military medicine and pediatrics; 1990–present for subanesthetic doses in acute and chronic pain; and from 2000 onward, psychiatric use showing rapid (within hours) and powerful antidepressant effects in TRD and acute suicidality. The mechanism involves induction of synaptic plasticity via the BDNF-TrkB-mTOR cascade. The approval of the S-enantiomer (esketamine, Spravato), new therapeutic targets (PTSD, OCD), and challenges (safety, abuse risk, pharmacoeconomics) are also covered.
