‘Shroom’ for concern: a case of psychedelic mushroom‐induced acute kidney injury

A man in his early thirties developed acute kidney injury 48 hours after ingesting 0.5 grams of dried Psilocybe cubensis mushrooms. He experienced flank pain and a rise in serum creatinine from 83 to 189 μmol/L, with bland urinary sediment and no other nephrotoxic exposures or dehydration. Supportive intravenous fluids resolved symptoms within 48 hours, and kidney function fully recovered by one month. Psilocybin-containing mushrooms are not traditionally linked to kidney damage, but their serotonergic activity may cause vasoconstriction and tubular injury. As psilocybin prescribing expands, clinicians should be aware of potential renal effects.
