Ketamine and Esketamine for Bipolar Depression

Ketamine and esketamine are promising treatments for bipolar depression. In a study of patients receiving twice-weekly treatment, 40% responded and 13% achieved full remission. No immediate manic switches occurred during the intensive phase, but 30% experienced hypomania or mania during the maintenance phase, most episodes mild. Involving family members in monitoring mood changes is recommended. Patients with a history of severe mania or rapid cycling may not be ideal candidates.
