Too Hot to Handle: A Case Report of Extreme Pyrexia After MDMA Ingestion

A 16-year-old male survived severe hyperpyrexia exceeding 43°C after taking ecstasy (MDMA), a condition usually associated with poor prognosis. The patient developed multiorgan failure and disseminated intravascular coagulopathy. Rapid cooling using an endovascular catheter, treatment of autonomic symptoms, fasciotomy, and multiorgan support in an intensive care unit led to a good outcome after four weeks. Survival from MDMA-induced extreme hyperpyrexia is possible with prompt cooling and comprehensive organ support in an intensive care setting.
