The neuropsychopharmacology of cannabis: A review of human imaging studies

Cannabis and its primary psychoactive component, THC, acutely alter executive, emotional, reward, and memory processing by directly interacting with the endocannabinoid system and indirectly affecting glutamatergic, GABAergic, and dopaminergic systems. Cannabidiol, a non-intoxicating cannabinoid, may offset some of these acute effects. Heavy repeated cannabis use, especially during adolescence, is associated with adverse effects that increase the risk of mental illnesses such as addiction and psychosis. This review synthesizes human neuroimaging research on the acute and chronic neuropsychopharmacology of cannabis, covering effects during development, implications for psychosis and cannabis use disorder, and methodological considerations.

Acute and chronic effects of cannabinoids on effort-related decision-making and reward learning: an evaluation of the cannabis ‘amotivational’ hypotheses

Daily use of high-potency cannabis is associated with more positive symptoms in first-episode psychosis patients: the EU-GEI case–control study

Daily use of high-potency cannabis is associated with more positive symptoms and fewer negative symptoms in first-episode psychosis patients. Among 901 patients and 1,235 controls across six European countries, those who used high-potency cannabis daily had the highest scores on positive symptoms, while negative symptoms were more common in patients who never used cannabis. Psychotic experiences in controls were linked to current cannabis use but not to lifetime use. Depressive symptoms showed no differences related to cannabis use in either group.

Dissociable effects of cannabis with and without cannabidiol on the human brain’s resting-state functional connectivity

Two strains of cannabis—one containing only THC (8 mg) and another containing THC plus CBD (8 mg THC + 10 mg CBD)—were compared with placebo in seventeen healthy, experienced but non-regular cannabis users. Using resting-state fMRI, both strains reduced functional connectivity in the default mode and salience networks relative to placebo, but with distinct spatial patterns. The THC-only strain specifically disrupted the posterior cingulate cortex within the default mode network, and this disruption correlated with subjective feelings of being 'stoned' and 'high'. The THC-plus-CBD strain produced less disruption of the salience network, suggesting CBD may buffer THC's effects on salience processing, which could underlie its potential therapeutic role in disorders like psychosis and addiction.

Cannabis Dampens the Effects of Music in Brain Regions Sensitive to Reward and Emotion

Cannabis dampens the brain's response to music in regions involved in reward and emotion, including the auditory cortex, hippocampus, amygdala, and ventral striatum. However, when cannabis also contains cannabidiol, these dampening effects are offset, and brain activity does not differ from placebo. Despite reduced neural responses, both types of cannabis increased participants' self-reported desire to listen to music and enhanced sound perception. The findings suggest that cannabidiol may mitigate some of cannabis's effects on brain reward systems.

Investigating the interaction between schizotypy, divergent thinking and cannabis use

Cannabis acutely increases schizotypy and chronic use is linked to higher rates of psychosis. Creative individuals tend to have higher schizotypy, but the connection between cannabis, schizotypy, and creativity had not been studied. In 160 cannabis users tested sober and intoxicated, cannabis increased verbal fluency in low-creative individuals to the same level as high-creative individuals. Cannabis also increased state psychosis-like symptoms in both groups. High-creativity participants had significantly higher trait schizotypy, but this was not linked to the verbal fluency change. Acute cannabis use enhances divergent thinking, measured by verbal fluency, specifically in those with low trait creativity.

MDMA (ecstasy) and compassionate imagery both increase self-compassion and reduce self-criticism in recreational users. In a non-blind experiment, participants who consumed ecstasy showed similar pro-social effects to those produced by a contemplative compassion exercise, particularly in those with higher attachment-related avoidance. The findings suggest MDMA may enhance psychotherapy by fostering compassionate attitudes toward oneself. However, because the study was not blinded and drug purity was unknown, controlled trials with pharmaceutical-grade MDMA are needed to confirm these effects.

The acute effects of cannabis with and without cannabidiol in adults and adolescents: A randomised, double‐blind, placebo‐controlled, crossover experiment

Adolescent cannabis users are neither more resilient nor more vulnerable than adult cannabis users to the acute psychotomimetic, verbal memory-impairing, or subjective effects of cannabis. In a randomized, double-blind, placebo-controlled crossover experiment with 24 adolescents (16-17 years old) and 24 adults (26-29 years old) who used cannabis 0.5-3 days per week, vaporized THC (8 mg for a 75 kg person) and THC plus CBD (24 mg) both significantly increased subjective drug effect, impaired verbal episodic memory, and increased psychotomimetic effects compared to placebo. There was no evidence that adolescents differed from adults in their responses, and CBD did not modulate the acute effects of THC.

Additive Effects of 3,4-Methylenedioxymethamphetamine (MDMA) and Compassionate Imagery on Self-Compassion in Recreational Users of Ecstasy

In a naturalistic setting, recreational users who consumed chemically verified MDMA performed a self-focused compassionate imagery exercise on two separate occasions: once after taking MDMA and once without. The exercise and MDMA each independently increased emotional empathy toward critical facial expressions and self-compassion, and their effects on self-compassion appeared to add together. The findings suggest that combining MDMA with compassionate imagery may enhance self-directed affiliative feelings, though controlled studies are needed to confirm the combined utility for therapeutic contexts.

Acute effects of cannabis on speech illusions and psychotic-like symptoms: two studies testing the moderating effects of cannabidiol and adolescence

Inhaling cannabis reliably increases psychotic-like symptoms in healthy cannabis users and may raise the incidence of speech illusion—hearing words in meaningless white noise. Cannabidiol (CBD) did not blunt these effects. In one study, adults had stronger psychotic-like symptom responses than adolescents, suggesting adolescents may be less vulnerable to the acute psychotic-like effects of cannabis. The odds of experiencing speech illusion were 3.1 times higher after inhaling cannabis with THC and negligible CBD compared to placebo.

The acute effects of cannabidiol on the neural correlates of reward anticipation and feedback in healthy volunteers

A single 600 mg oral dose of cannabidiol did not alter brain activity related to anticipating or receiving rewards in healthy adults. Using functional magnetic resonance imaging during a monetary incentive delay task, the expected reward-related brain regions—including the insula, caudate, nucleus accumbens, anterior cingulate, and orbitofrontal cortex—were activated, but no difference was observed between cannabidiol and placebo. Bayesian analyses confirmed that activity in these regions was similar under both conditions, and behavioral measures of motivation for reward also showed no significant difference. The findings suggest that acute cannabidiol does not affect the neural correlates of reward anticipation or feedback in healthy individuals.

Substance Use and Attendance Motives of Electronic Dance Music (EDM) Event Attendees: A Survey Study

Attendees of electronic dance music events are a high-risk group for substance use. A survey of 1345 Belgian attendees found that ecstasy/MDMA/Molly (52.28%), other synthetic hallucinogens (53.68%), ketamine (42.13%), amphetamines (40.45%), and alkyl nitrites (32.76%) were most used at festivals, outdoor parties, and raves. Cocaine was prevalent in nightclubs (32.29%), while cannabis (68.88%) and magic mushrooms (66.44%) were most used at private events. Overall enjoyment was the key motive for attendance, followed by music and socialization. Users rated many motives (dance, exploration, escapism, excitement, alcohol, drugs) as more important than non-users. Substance use prevalence depended on the event setting, and a three-dimensional classification of attendance motives was supported.

Individual and combined effects of cannabidiol and Δ 9 -tetrahydrocannabinol on striato-cortical connectivity in the human brain

THC strongly disrupts connectivity between the striatum and cortex, but co-administering CBD mitigates this effect in the limbic striatum network. In one study, inhaled cannabis with 8 mg THC or 8 mg THC plus 10 mg CBD disrupted associative and sensorimotor networks, while THC alone also disrupted the limbic striatum network. In a second study, oral 600 mg CBD increased connectivity in the associative network and caused minor disruptions in limbic and sensorimotor networks. The insula emerges as a key region affected by cannabinoid-induced changes in functional connectivity, with implications for understanding cannabis-related disorders and developing cannabinoid therapeutics.

Effects of Cannabidiol and Delta-9-Tetrahydrocannabinol on Plasma Endocannabinoid Levels in Healthy Volunteers: A Randomized Double-Blind Four-Arm Crossover Study

Inhaling vaporized cannabis containing 10 mg of THC acutely increased blood levels of the endocannabinoid anandamide by 18% and several related noncannabinoid lipids, but adding 10, 20, or 30 mg of CBD did not alter these effects. Over four sessions spaced about two weeks apart, pre-inhalation levels of anandamide and one related compound progressively declined, possibly due to repeated THC exposure or reduced anxiety. The findings suggest that CBD, at the doses tested, does not modulate THC's acute influence on endocannabinoid concentrations, though higher or chronic CBD doses might have an effect.

Acute effects of different types of cannabis on young adult and adolescent resting-state brain networks

Cannabis use disrupts resting-state functional connectivity in key brain networks—default mode, executive control, salience, hippocampal, and limbic striatal—similarly in adolescents (16–17 years) and young adults (26–29 years). Cannabidiol (CBD) did not counteract the effects of delta-9-tetrahydrocannabinol (THC) and, in some cases, further reduced connectivity both within networks and across the whole brain. These findings challenge the assumption that CBD makes cannabis safer and indicate that the adolescent brain is not more vulnerable to cannabis-induced connectivity disruptions than the adult brain.

Associations between regular cannabis use and brain resting-state functional connectivity in adolescents and adults

Regular cannabis use is associated with localized increases in connectivity of the executive control network (ECN), which may reflect adaptive or compensatory changes in response to regular intoxication. Adolescents generally showed weaker connectivity than adults across multiple brain networks, consistent with normal developmental effects. However, there were no significant interactions between age group and cannabis user group, meaning the connectivity changes linked to cannabis use did not differ between adolescents and adults. No associations were found between cannabis use frequency and any connectivity measures.

Does variation in trait schizotypy and frequency of cannabis use influence the acute subjective, cognitive and psychotomimetic effects of delta-9-tetrahydrocannabinol? A mega-analysis

Tolerance to some acute effects of delta-9-tetrahydrocannabinol (THC) develops in a domain-specific way, with more frequent cannabis use linked to reduced intensity of subjective effects like feeling stoned and reduced psychosis-like effects. Tolerance to alertness reduction occurred more readily than tolerance to psychotomimetic effects, while tolerance to memory impairment was weak. Trait schizotypy did not moderate the acute psychosis-like effects of THC. These findings come from a mega-analysis of 128 cannabis users across four double-blind, placebo-controlled studies.

The acute effects of cannabis, with and without cannabidiol, on attentional bias to cannabis related cues: a randomised, double-blind, placebo-controlled, cross-over study

Acute intoxication with vaporized THC eliminated attentional bias away from cannabis-related cues in both adolescents and adults who used cannabis 0.5–3 days per week. In a randomized, double-blind, placebo-controlled crossover study with 48 participants (24 adolescents aged 16–17 and 24 adults aged 26–29), a weight-adjusted dose of 8 mg THC eliminated the bias, while adding 24 mg CBD did not alter this effect. There was no significant difference in response between age groups. The findings suggest that acute THC intoxication shifts attentional processing of cannabis cues, but CBD does not moderate this effect.

Acute effects of Δ9-tetrahydrocannabinol (THC) on resting state connectivity networks and impact of COMT genotype: A multi-site pharmacological fMRI study

THC, the main psychoactive component of cannabis, reduces functional connectivity in the salience network—a brain system involved in processing awareness and salient information—specifically weakening connections from the right insula to the left insula and anterior cingulate cortex. A trend toward decreased connectivity was also observed in the hippocampus-midbrain-striatum network, which may relate to acute psychotic-like effects. The COMT Val158Met genotype modulated subjective responses: individuals with the Met/Met genotype experienced the strongest dysphoric reactions and showed reduced connectivity in the hippocampus-midbrain-striatum network after THC. These findings suggest that prefrontal dopamine levels influence the acute effects of cannabis.

Receptor-enriched analysis of functional connectivity (REACT) for understanding cannabinoid neuropsychopharmacology

Inhaling cannabis with THC alone or THC plus CBD reduces resting-state functional connectivity in brain networks that are rich in CB1 and CB2 cannabinoid receptors. Co-administering CBD with THC produced more widespread reductions than THC alone, affecting regions involved in cognition and emotion such as the prefrontal cortex, cingulate cortex, insula, hippocampus, amygdala, and putamen. Higher THC levels in the blood were linked to greater connectivity decreases when CBD was also present. Subjective drug effects correlated with connectivity changes in a region-specific way. The findings suggest that CBD may enhance rather than dampen THC's effects on brain connectivity, contrary to some earlier expectations.

THE EFFECTS OF ACUTE CANNABIS WITH AND WITHOUT CANNABIDIOL ON NEURAL REWARD ANTICIPATION IN ADULTS AND ADOLESCENTS

In weekly cannabis users, cannabis suppresses the brain's anticipatory reward response to money, and cannabidiol (CBD) does not moderate this effect. The adolescent reward circuitry is not differentially sensitive to the acute effects of cannabis on reward anticipation. This double-blind, placebo-controlled, crossover study with 23 adults (26-29 years) and 24 adolescents (16-17 years) used functional magnetic resonance imaging (fMRI) to measure brain activity during a monetary reward anticipation task after inhaling cannabis with tetrahydrocannabinol (THC), THC plus CBD, or placebo. THC acutely reduced reward anticipation activity in the ventral striatum and right insula compared with placebo. THC plus CBD also reduced activity in these regions. The effects were the same in adolescents and adults.

Cannabis use as a potential mediator between childhood adversity and first-episode psychosis: results from the EU-GEI case–control study

Childhood adversity and cannabis use are both known risk factors for psychosis. This study examined whether cannabis use might explain the link between childhood adversity and psychotic disorders. Data from 881 first-episode psychosis patients and 1231 controls from the EU-GEI study showed that household discord was associated with psychosis partly through cannabis use. Lifetime cannabis use mediated 17% of the association, cannabis potency mediated 14%, and frequency of use mediated 29%. Similar results appeared for early exposure to household discord. The findings suggest that harmful cannabis use patterns partially explain how household discord leads to psychosis, and children exposed to challenging home environments could benefit from interventions to prevent cannabis misuse.
