At least 23.5 million Ukrainian citizens are at risk of mental health deterioration due to the war, with up to 20% facing moderate to severe conditions and about 70% mild cases. Psychedelic-assisted therapy (PAT) using MDMA, psilocybin, and ketamine is described as a breakthrough treatment in the US, being three times more effective than standard psychotherapy and twice as effective as standard medication for PTSD, with shorter treatment duration and similar safety. In pre-war Ukraine, work began on theoretical foundations and personnel training for PAT, but current outdated legislation—specifically a 2000 Cabinet resolution banning necessary psychotropic substances and a 2008 Health Ministry order not accounting for combined therapies—prevents its implementation. The authors recommend urgent legislative changes to reclassify MDMA, DMT, psilocybin, and psilocin and to establish expanded access programs.
